Please complete Patient Information on the Hair Specimen Submittal Form. Follow instructions for taking hair sample. Place hair sample in a small, clean plastic bag. At least 1 tablespoon of new hair growth is needed for testing. Mail submittal form and hair sample with payment to:

Remember if hair has been cosmetically treated with bleach or permanent wave solutions, to wait 8-10 weeks before taking hair sample.
Take sample from first inch of new hair growth and cut off the rest and discard it.
